Release Forms

• Model Release Form

Required for the publication of any “portrait-style” pictures featuring an individual.

• Personal Data Publication Release Form

Required for the publication of any personal information, such as mundane name, email address, physical address, phone number, et cetera.

• Photograph Grant of Use Release Form

Required for all pictures published on the website. Note that a photographer may choose to grant perpetual rights and need to fill out the form only once.

• Photographer/Videographer Release Form

Alternative release required for all photographs or videos published on the website. Note that a photographer or videographer may choose to grant perpetual rights and need only fill out the form once.

• Creative Work Assignment / Grant of Use Form

Required for all original material submitted for publication, such as articles, poems, stories, et cetera. This form may also be used for original photographs and videos as an alternative to the photo or photo/video release forms.
